Middle School Physicals and Athletics Forms are online.

Starting in the 2024/2025 school year, all physicals and other athletic forms must be submitted online through the Aktivate platform.

Key Points:

• Physicals: Continue to get physicals from your local doctor or during the school's physical day in the spring. Physicals are valid for one year from the date of the exam.

• Online Submission: Register on the Aktivate website, upload a picture of your athlete's physical, answer questions about their medical history and insurance information, and sign other required forms. This system helps coaches quickly access vital information in emergencies.

• Current Physicals: Any physicals already submitted remain valid for one year. If a physical expires during the 2024/2025 school year, you must register on the Aktivate website and submit the required information online.

• No Cost: There is no cost for parents or guardians to use this platform.

• Deadlines: Be mindful of submission deadlines to ensure your athlete can participate in tryouts for all sports. Once registered, Aktivate will notify you of upcoming expirations.
