Aktivate: Athlete Online Registration
Physicals and Athletics Forms are online.
All physicals and other athletic forms for Morgan County Charter School Athletics must be submitted online through the Aktivate platform.
Key Points:
Physicals: Continue to get physicals from your local doctor or during the school's physical day in the spring. Physicals are valid for one year from the date of the exam, and must be uploaded to Aktivate. You can print the Physical Forms for your Doctor in English or Spanish, or download directly from Aktivate.
Online Submission: Register on the Aktivate website, upload a picture of your athlete's physical, answer questions about their medical history and insurance information, and sign other required forms. This system helps coaches quickly access vital information in emergencies.
Current Physicals: Any physicals already submitted remain valid for one year. If a physical expires during the 2025/26 school year, you must register on the Aktivate website and submit the required information online.
No Cost: There is no cost for parents or guardians to use this platform.
Deadlines: Be mindful of submission deadlines to ensure your athlete can participate in tryouts for all sports. Once registered, Aktivate will notify you of upcoming expirations.
